As older adults' health and mobility worsens, getting to a doctor's office can become very challenging. The health of older adults can be unpredictable and they need doctors that can respond quickly to their needs instead of booking them appointments weeks from now. If they live in assisted living facilities, the doctors need to work closely with the staff to make sure things happen smoothly. X-rays, ultrasounds, and bloodwork can be ordered to take place in the home setting, so the vast majority of issues can be handled with convenience.
Studies have shown that patients who become ill and get hospitalized can take a long time to get back to their old selves. During the 30 days after discharge, patients can be especially vulnerable to rehospitalization. Many causes for rehospitalizations are preventable, but the problem may be as simple as getting seen by a doctor during that time. There are changes to medications, follow-up appointments, getting used to new disabilities and finding caregivers are all potential reasons to seek help at an emergency room again.

Mary is an 85-year-old woman who lives alone and is cared for by a caregiver at home. She can do her ADLs but she walks with a walker because of balance issues and she cannot cook or clean for herself. She has a vague history of COPD but doesn’t take medicines and hasn’t been to the hospital or seen a doctor in years. She is 5’ 2” and 55 kg (121 lbs). Her caregiver has noted that Mary is eating less than her usual plate over the past month.
According to the SNAQ-RC malnutrition screening tool, Mary is at high risk for malnutrition. She is at increased risk of having more trips to the hospital, longer stays in the hospital, and earlier death.
Involuntary Weight Loss is a serious problem for seniors. It can be caused by 4 major issues: 1) Inadequate dietary intake; 2) Appetite loss (anorexia); 3) Muscle atrophy (Sarcopenia); 4) Inflammatory effects of disease (cachexia)
With the help of caregivers, increased attention to diet and exercise can help address some of these issues. Most seniors who have this problem do not recognize it and they often need help to even start addressing the issue.

For adults under age 65, a Body Mass Index (BMI) of over 22 is considered normal, but for older adults a BMI of 22 is associated with increased death. In fact, being overweight is associated with less mortality in seniors.
Decreased appetite, decreased sense of smell and taste is often a normal part aging but medical conditions should be ruled out that might be causing these problems.
Older adults also tend to start eating less protein while maintaining their carbohydrate intake, so they need to be very mindful about taking enough protein to protect their muscles. Taking in at least 1 g/kg/day (55 g for Mary), may help decrease loss of mobility.
Exercise in frail older adults helps maintain and gain weight, not lose weight.
